Patient Safety and Experience

The Patient Safety and Experience theme looks at patient safety from a system perspective, involving staff, carers and patients. We pay attention to the detail, explore innovation that is evidence based and patient-centered, and connect people with shared problems in an atmosphere that generates solutions to their challenges.

Through exploring new ideas, sharing great practice for making care safer and focusing on what matters to patients, we aim to create positive healthcare experiences for all.

Patient Safety improves the experience of everyone by allowing the system to learn more about itself and be discussed in an open and honest way.

Catherine Dale

Programme Director, Patient Safety

Catherine has nearly twenty years’ experience in the NHS in London, including more than fifteen years in service improvement and transformation roles; a Masters in Business Psychology and expertise in Experience-Based Co-Design.

Adrian Hopper

Clinical Director, Patient Safety

Adrian Hopper is physician with a special interest in Geriatric Medicine at Guy’s and St Thomas (GSTT) NHS Foundation Trust in London. He is currently Deputy Medical Director for Patient Safety at GSTT, the lead for the Medicine Clinical Academic Group for King’s Health Partners and Clinical Director for Patient Safety and Clinical Lead for Healthy Aging at the HIN.

Cleo Butterworth

Associate Clinical Director, Patient Safety

Cleo is currently the Associate Clinical Director for Patient Safety at the Health Innovation Network. Her ambition is to promote collaborative and partnership working across traditional boundaries for sustainable change. With a background in pharmacy she is passionate about medicines optimisation and its potential to deliver truly patient-centred, safe and effective coordinated care.

Formerly a copy-editor for scientific journals and news, Jamie moved over to the Health Innovation Network in September 2017. He now supports the Patient Safety and Experience Team and is involved in a range of projects from Catheter Care to Health and Wellbeing and Communities of Practice.
