Healthier You reaches south Londoners at high risk of diabetes

For the last few months south Londoners have been referred to Healthier You, the National Diabetes Prevention Programme. The course is run over nine months as a behaviour change programme for people identified as at high risk of diabetes.

For the last few months south Londoners have been referred to Healthier You, the National Diabetes Prevention Programme. The course is run over nine months as a behaviour change programme for people identified as at high risk of diabetes.

Participants learn about the importance of exercise, good nutrition and maintaining a healthy weight. The 24 CCGs and local authorities have come together in partnership to deliver this programme for south London, aiming to reach over 9000 people over the next two years. The HIN coordinated the initial bid for south London and is working closely with the lead CCG, Southwark, to report progress to NHS England.

To date, partners have referred over 1700 people into the programme and counting. At the moment we have participants attending 25 programmes across south London, with plans for more programmes starting at the beginning of January.

Partners have used different approaches to identifying and referring people. We are already looking at how to share best practice and lessons learned across the partnership, so that we can keep up the momentum.
