Please use and adapt this leaflet for referrers into the DESMOND programme. People with Type 2 diabetes tell us that frequently not enough information about the course is given to them at referral and the benefits of the course were are not ‘sold’ to them. Many health care professionals won’t know what DESMOND is and how essential it is for their patients with diabetes to attend. The leaflet outlines the importance of people with diabetes attending DESMOND and signposts them to resources like video links where they can learn more.
