PSIRF in General Practice

In June 2024, the Health Innovation Network South London (HIN) began a pilot programme to introduce the Patient Safety Incident Response Framework (PSIRF) to general practice (GP). The work was commissioned by NHS England’s Patient Safety team. The approach builds on learning about the implementation of PSIRF in secondary care and other settings over the past few years.


The PSIRF sets out the NHS’s approach to developing and maintaining effective systems and processes for responding to patient safety incidents. It is for the purpose of learning and improving patient safety. Key principles to PSIRF are:


  • Compassionate engagement.

  • Systems-based approach.

  • A proportionate response.

  • Supportive oversight.


56 organisations from across England joined the pilot, including Primary Care Networks (PCNs), GP Federations, and Integrated Care Boards (ICBs). These organisations, known as early adopters,  tested resources and approaches; attended monthly webinars; and shared their learning. The approach and learning have been captured in the End of Pilot Report.


In April 2025, the HIN launched the second phase of testing PSIRF in general practice. The focus of this work is to test how different parts of the healthcare system can work together to keep patients safe. It will build upon the learning from year one, with resources and learning to be shared in March 2026 to support wider implementation across primary care.
