The guidance packs support providers in South East and South West London who have received a FeNO device through the PTF project. They include information around data collection, training, patient prioritisation, evaluation and further implementation resources.
Please read thoroughly to understand the asks of your service and to understand how we at the HIN can support you to successfully deliver and evaluate this project.
The following resources are to support the project evaluation. Please note patient surveys can be completed online via Microsoft Forms or on paper. More information can be found in the Patient Information Sheet below.
If you have completed paper patient surveys, please collate at the end of each month, scan and email to pearl.brathwaite@nhs.net
Community providers without primary care system access will need to use a separate manual data collection template.
